The amount of water wasted when a toilet ghost flushes depends on the cause of the ghost flushing. Leaky flapper valves tend to waste less water than a larger problem like a damaged flush valve that makes the toilet run constantly. Yes, you should screw your flange to the subfloor, including cement. The best way to get this done is to predrill holes into the concrete using a hammer drill and a ¼-inch concrete bit.
