A door frame is not the same as a door casing. A door frame is the framework surrounding the entire door that supports the area from which the door hangs. The frame is made up of multiple parts, including the door jamb, head, and sill.

Casing refers to the trim work located around the door frame. It connects the door frame to the wall with decorative trim.

Permits are required when carrying out any structural work, so in most cases, you will need a building permit to cut through a concrete block wall to install a door. The vast majority of hinges are symmetrical, and either side of the hinge can go on the door or the frame. Occasionally, you'll find a hinge specifically made for right or left-handed doors with different attachment hardware or larger plates on one side. It is possible to install hinges backward, so be careful when placing them. Position the hinge so that the hinge pin faces the direction the door will swing. For example, if you're standing on the side the door will open into, the pin should be visible. Be extra careful when installing exterior doors, as placing the hinges on the outside for an outward-facing door makes your home much easier to break into.

You want a small gap of 1/8 of an inch between the door and the jamb. However, the gap size depends on the direction the door swings and whether it contains glass. You will also want to aim for the same size gap on all sides of the door, so it opens and closes smoothly.
