Tiling:  We had three different sizes of the same tile that was to be used for the shower and tub deck area.  The wall tile was installed and bullnose tile was started and no one had checked that the dye lots were the same or noticed that they color of the tile was different between the two sizes.  We had to bring this to both the tiler and general contractor and have them remove the bullnose tile after install.  Also the tile we were using was hand dyed so had some variation in pattern across the tile.  A circular pattern was installed that made it look like we had a bulls-eye around one of the shower heads.  This tile was removed and reset, and a number of other tile was not installed flush and had to be removed and reset.  Also they did not look at the install instructions for around the shower controls and left a large gap around the control mechanism and did not install the waterproof membrane that was provided.  We had to ask to have this done as the gap was larger than the trim, and the manufacturer warranty would be violated if they did not use the included membrane.  These instructions and membrane were given to them on day one of tiling, and they did not use.  Also 2 months after the tiling was completed and we are having cracking in the grout joints.
Hardwood floor refinishing:  Due to other damage caused during this project, they were asked to refinish the 2 existing rooms and lay new hardwood in the new space.  Previous refinishing of the hardwood floors were done in 2005 and used a high grade finish.  At no time did they ask for information other than ask if this was a gloss finish and oil or water base?  We would have preferred to have the same finish used as before.  The floor guys were scheduled for 1 day of work.  They sanded one day, and came back the next day to finish.  But did not finish that day, no finish has been put on the floor.  Ended up coming back a third day, Saturday, to put the finish coats on.  They did not keep the PM informed and we came home Friday to an unfinished floor and no sign or word that they weren't finished or when they were coming back. There were a couple of boards that were rough to the touch and a couple of new wood boards that have a number of small holes, that we had to have them come back out and resolve.
Also when they came to touch up from some issues after the tub was installed, they used a different finish than they applied to the floor with the refinishing.  The person who touched up the floor told the project manager he would have to come back the next day to sand, and then left.  When talking with the project manager and noticing that he didn't even use the same finish, we called the store to try to get him back and answer more questions.  They reached out to the guy and found out he would have to do more than just sand when he came back.  He had also sanded some patches of floor in the hallway which were not part of the project space, but had issues from the tub install, and applied the same finish product as he used in the bedroom, wrong finish and wrong type of product that doesn't work with the existing floor finish.  At this point, I asked that these people not come back, as they are causing more harm that fixing.  We will have the floors re-done with the same contractor as did them in 2005.
