The provider used a fill in form.  There were no additional details beyond the form though so check out the form first and see if it satisfied what you are looking for.  
 The person was very very nice, came very promptly.  After inspection I had alot more questions and my phone calls were not returned and email was not returned and we were waiting on closing.   I wanted to know if the appliances were functioning, furnace, air conditioning etc.  Also I wanted to know if the water was working in the bathrooms and kitchen.  Turns out the down stairs is not airconditioned at all and was advertised as whole house air.... no vents :)   and downstairs bath was leaking really badly and water was off.  Also, there was damage to outside of house where a car hit it but none of that was in report.    He did report on many things that were done well.  The agent even tried to contact him.   We ended up driving up to do rest of inspection ourselves.  I was just disappointed that once he inspects that's it, you can't ask any more questions or get any more info beyond what is on the form.
