October 2011 - Preparation work conducted; asphalt applied.
October 2011 - Driveway and Pull Off: Realized that edges in particular areas looked uneven/lumpy and did not appear to harden like the remainder of the driveway; Warren Lane: The approximately 1/4 mile long private road appeared to be done well.
November 2011 - First cracks appeared along back edge of pull off (about 12 inches x 10 feet) and along a portion of the driveway edge (about 4 inches x 40 feet -- this was beyond the edge of the original driveway and so did not have any underlayment); Placed call to contractor to express concern and did not receive a response
December 2011 - Did not receive response from contractor; Placed second call; Received call from contractor in late December and was told that the asphalt production had halted for the year and he would contact me in the spring; I informed him that the reason for the crack appeared to be placement of asphalt over grass/soil that had not been laid over compacted stone as had been done over the majority of newly paved areas.  Contractor did not acknowledge this error and instead blamed me for cracks due to vehicles running over the edge.
May 2012 - Called contractor, waited two weeks, called contractor again; Contractor "fixed" cracks by adding very small amounts of asphalt (literally about 1-2 tablespoons) and melted it into the cracks.  This "fix" did not address the underlying problem of laying asphalt over a non-solid base (grass/soil) and it is clear that cracks will quickly reappear. 
The contractor was unable to realize that everything his crew performs wasn't perfect.  He continually repeated his level of experience while finding excuses as to why any problems were not his fault.  My concerns were met with an attitude of being offended rather than trying to find a workable solution.  The contractor does not operate in a professional manner.
