Contacted three vendors.  One kept postponing coming out to give an estimate, one never called, and then Punch-List came out.  The price was about what I expected ($1,000 for 225 sq. ft.).  Initially the floor looked great.  Then I found out that to level the planks, they used cardboard.  Not crazy about the idea, but it seemed to work.  After two days, the work was complete and I got started back on cabinet installation.  During that time, I began to notice that there were a couple of planks that seemed rather bouncy.  Near the laundry room, I noticed also that they had completed the corner with a very small piece (smaller that recommended by any floor installer).  It has since come loose.
The guys that installed the floor were quick, professional, and worked hard.  I'm not sure if they had much experience with floor installation, and it shows in some aspects of the work.
Also asked for an estimate to replace the wood on a porch, they said they'd come back.  Not yet.
