Hair Salons/Barbers
The manicure seemed fine initially but that same evening, I noticed that every single nail had bubbled.  Unfortunately, it was too late to get the problem fixed.  I literally spent the entire weekend at two parties hiding my hands because they were embarrassing.  Still, I didn't remove the polish because I wanted to show the salon what had happened.  
  
When I got back into town, I went back to the salon. When I showed them my nails, they immediately knew which beautician had created the problem. If they knew she would cause problems, why did they assign her to me in the first place? And they also knew what the problem was. Gee, maybe more training of her could have avoided the problem.
  
I asked for a refund but instead got an offer for another manicure with a different beautician. When I refused, because I no longer trusted them (after all, you get only one chance to create a good first impression), I was told I didn't have "an open mind," not a very customer oriented response.
  
I gave them my name and address for a refund. Six days later, nothing in the mail.
  
Update: as of January 3, 2012, a refund check arrived from Geneses.
