At about the 10-year mark there was a partial failure with the caulking. I had no idea how to correctly repair it myself so I called Midwest. They came out and removed the bad caulk and repaired it in about 15 minutes - at no cost. That's 10 years later and no charge to drive down 45 miles and fix it.

Update:
The caulking around the tile is loose and is coming out around the tile in the kitchen (backsplash). It has been approximately six months since it has been done and I am having to have someone else come out and fix it. The pocket door between the bathroom and bedroom right now it is not working at all, I can not pull the door out to use it.
Update 2 (9/22/2014): I have noticed that the caulking is coming up around the toilet and other areas in the bathroom when I clean the floor. The caulking does not do well with moisture and is dwindling away each time clean. I am contacting the provider today to see what else can be done.
Update: 3. (9/29/14). I had a plumbing company check the tiles around the toilet. There was water under the tiles almost up
To the vanity. As I said above the grout (caulking) is coming out. The toilet had to be pulled & work done around the seals. Also there were broken tiles and the toilet had to be made level.
The tile is uneven around the toilet as well as the entrance to the bedroom door. I talked to the owner Marcy after the previous update
On 9/22/14 and when I told her I would like to have a $1000 for all the extra expenses & convenience we have had with this project, all she will do is tell me that she will have someone to come out & take care of it. This was all before the expense I had yesterday. I do
Not want her crew to fix anything in my home again. I wish I had never had Midwest Luxury Bath to do anything. I had hoped for a custom bath, which of course is not the case. I told the office lady Tia that the owner could come out & look to see what it looked like but
I would not allow them to do the work.
"Every effort was made to satisfy the (removed member name). There were several products that they wanted that were unattainable (Toto toilet that had been pulled out of production due to poor flushing issues.) This has been a summer of difficult supply issues, and we worked as best we could to work through theses issues. This was a one bathroom home with one of the client's being very ill in the middle of it all and us trying to keep them in a working bathroom at all times. (removed member name) is very particular and we stayed with the project until she said she was happy with it. "The property where the work was completed is one of the member rentals. We do provide cleanning and care instructions to all our customers at the time of completion of the work and they are also available on our web page. Apparently the tentes had change and the new tentes did not receive the care instructions and in turn used a drain cleaner and other chemicals which were not to be used causing the liner to crack. Due to chemicals being used that were on the "don't use" list of the care instructions the warranty was voided. Tring to help the member through this unfortunate incident we offered to replace the liner at cost. Since the issue was not something we had control of we felt providing the materails and labor at no profit to us was reasonable. We could not pay for a mistake that we had no control of and could of been avoided. But did offer a reasonable solution."
