 Timeline of Events:
 We met Mike at our
office May 15, 2014 so he could take pictures and get a feel for what we wanted
for a wrap for the Honda Odyssey van. Mike mention there was a 5 year warranty
on the wrap.
 Deposit was paid June
14, 2014 in the amount of $200.00
 We received first proof
June 17, 2014 via email
 July 26, 2014 the final
proof was accepted
 Another payment was
made of $1000.00 on July 30, 2014
 Scheduled application
of vehicle wrap August 13, 2014 then changed to August 18, 2014
 Location for drop-off:

We arrived dropped the
van off, Mike called us once we had returned to the office. One of the installers
from (American Install Company, LLC- owner Jim Cobb- 468-8832) stated that the
van needed some painting where there was primer on the vehicle. We had to go
back and retrieve the vehicle. Then numerous other events occurred that delayed
the process like: One of the installers had words with Greenwood RV Rentals
employees (where the wrap was being applied), and they were asked to leave. Dream
Street was then added to the equation and asked to complete the wrap.
 Date for installation
was changed to September 3, 2014 ? same location as above.
 We sent email letting
Mike know the slogan was missing on the back of the van.
 Around September 19,
2014 the slogan on the back of the vehicle was corrected.            
 Final payment was made
October 7, 2014 in the amount of $1140.00
 At the end of January
we noticed the wrap peeling off the back and side (passenger doors), and around
side window (passenger side)
 We emailed Michael
Linginfelter Suyeyasu, February 2, 2015: We informed him that the wrap was
peeling off. He informed us ?He was no longer in business? and he stated in his
email ?I can tell you that Jim Cobb @ American installs did the majority of
work on the Honda Odyssey.  I worked with
Tim Bettis @ Dream Street fix Jim's work.?
 We proceeded to call
Sign-a-Rama Corporate ? They gave us the name of Joel K. to contact for this
issue @ 561-868-6362 (a couple weeks later we were told Joel no longer works
there). We were given another person to contact Harry Skeem @ 561-640-5570;
Harry proceeded to tell us he would look into it and see what they could do.  We were then contacted by Frank Pisano
561-868-1469 and he asked that pictures be sent of the vehicle and the paid
invoice. We complied with this request on March 11, 2015 via email. At that
time he stated ?we may have someone out to take some pictures?, He did not know
what they could do but he would look into it.

Cell 3179036938 - from Carmel franchise called and left message stating that he
wants to make an appointment to see the van and see what it will take to repair.

I called to speak to
Travis and confirm apt for today. He was not in the office when I called
8:30am. I spoke to someone else and he transferred me to Travis?s voicemail. I
left a detailed message to confirm apt for today at 1:30pm. Travis will be
looking and taking pictures while van is onsite in Zionsville at customer?s
location, due to Travis not want to travel all the way to Greenwood.

was waiting at location for 1 hour. I returned his call and told him the
appointment with us was yesterday. We reset an appointment for 05/07/2015 ?
between 9-11am.

office Travis not in, Called cell no answer left message in regards to the
outcome of his visit to look at the van on 05/07/2015? Asked him to call and if
I was not in to speak to Robert and that I would be leaving for vacation and
not returning until the 26th.
