**UPDATE**
After a bit of a rocky start, Chad came in and knocked out the job. We are pleased with the result and with the quality of the work. Time will tell about the durability of the product (micro-topping), but we are satisfied with the look and feel of the product.
Underestimated the quantity of concrete, and as such had to order/pay the concrete supplier directly for a second truck.
Chad's people did a very poor job at stamping in many places as well as uneven coloring of the concrete. Multiple sections where not enough release was applied and caused suction marks on the concrete. In addition, many cracks in the surface concrete due to improper curing. As a professional concrete company, they should know exactly how to prevent this type of cracking if the surface is drying to fast.
We've paid Chad in full with a promise that he would return two weeks later to fix sections where cracking and uneven colorization occurred. As well as final cleanup issues which were not addressed the last time they were here.
We have left multiple emails and voicemails over the past 2 months with absolutely no call backs. It is obvious Chad and his company have no intention of coming back. We'll have to hire another concrete company to fix the poor quality of Chad's work. We only hope that we can recoup some of this additional cost after filing in small claims court against Chad.

you can expect if you make the mistake of hiring Creteworx, or whatever company Chad starts back up if this one is now defunct.
"[Member Name], We provided an estimate we exceeded it by only 9%. You added over $1500 in change orders. We ran short of concrete and I told you I would need some help covering these extras and I asked you to call the concrete company and provide them with your credit card company (I used all of our allowance). Instead you wanted me to give them your credit card number which I did, then you decided to your card company and charge the concrete back as fraud? Yes, if you do not pay for your materials you will have a lien placed on your property by the ready mix supplier. I responded to you immediately via Angie's List member center - how can you dispute concrete that was delivered to your property? Prior to us pouring I asked that you inspect the concrete forms to make sure we covered all of our bases, unfortunately you claimed we missed an area of 24 sf that I offered to fix it after you paid us the $600 you owed Creteworx, $1500 to concrete supplier. I would gladly pay for the spare dumpster and crush crete you authorized after you settled up with these costs. I have responded with the other 2 consumer outlets in the same manner. I will not move on my decision until these folks and Creteworx are paid to a zero balance. When you called fraud on your credit card that displayed enough of your character so I only returned the same courtesy to you. I am not sure what your intentions are but your fraud charge against Grizwold is an untrue statement and I am willing to testify on their behalf. By the way the the concrete total cost from my pocket exceed the allowance."
This has been an absolute nightmare. The job was completed on 1/20/14. I paid Creteworx in full the next day, 1/21/14. On 1/27/14 I received a Notice to Owner from his supplier stating that if the materials were not paid for that I could be held liable. I assumed this was standard procedure and called the supplier. They stated that his account had not yet been paid in full. I contacted Chad and he assured me that he would take care of the invoice. Approximately 1 month later I received a Notice of Lien from his supplier stating that a lien had been placed on my property. I immediately contacted Chad and he stated it was all a misunderstanding and he would take care of it.
After consulting with my Attorney I went to the Courthouse and filed a Contest of Lien, as his supplier did not follow the Florida Statute 713.13 in serving the Notice to Owner. Also, it is important to note that at no point in time did Chad ever bring up nor offer a Notice of Commencement (Statute 713..13) or a Release of Lien (Statute 713.20) Of course, being an average homeowner I did not know any of these were necessary. I was given only an estimate which I accepted, and an invoice which I paid in full. I strongly advise any homeowners to familiarize themselves with Florida Statutes 713.001 - 713.37 and it 's subsections before hiring a contractor.
As of today, 4/2/14 this matter is still not resolved. I have held off on this review to in the hopes it would be resolved. At this point I have no faith that it will be without intervention by not only the supplier's attorney, but my own as well.
I also discussed other jobs that I need done, driveway and some masonry repair on the home. Chad offered some great solutions and has good ideas. I would definitely recommend Creteworx for other jobs and I plan to use them when I'm ready to do those jobs.

Chad is very professional and listened and made recommendations on my needs and desires. It took a few days and a couple rescheduled appointments to get the estimate. But once a fair price was reached, was able to get the slab laid in less than a week.
I will update and finish the review once the project is complete.
Overall, they did a great job and I would hire them again. The job did take a bit longer than I expected, but it was quite a big job so that wasn't unexpected. The crew who actually did the concrete worked their butts off all day in the hot sun, and I was impressed with their work ethic. Creteworks did a good job and I would definitely hire them again.
