Description of Work: When I called to make an appointment, I asked them to fax to me copies of all documents that I would need to sign. I was told there was nothing but a one page intake form, which they did not fax to me. Again on the morning of the appointment (the appointment was at 2:45), I stated again that I wanted to have faxed to me all documents that I would need to sign. I received the same answer. since I was dubious of this answer, I persisted in questioning, and eventually, had 14 pages of documents faxed to me, which contained policies that I need to agree to. this included the doctor's policy to not accept calls from the patient's pharmacy for prescription refill requests and a requirement for URINE TESTING. In other words, the doctor does not trust her own patients and requires them to undergo urine testing to screen for illicit drugs. needless to say, after being lied to about the existence of these policies in the first place, as well as the policies themselves, I canceled the appointment.
