Mr. Moquin and his assistant showed up a bit early; but, that was ok. He helped me moved a couch from the family room. He and his assistant then proceeded to removed a wall-to-wall carpet and padding. They had to "shave" some adhesive from the slab that flowed over from the tile floor in the dinette, as well as other stuff that would affect the smoothness of the slab. Then they vacuumed the area.
Once that was done, they proceeded to lay down a vapor barrier, essentially a huge piece of plastic, to cover the slab where the floor was being installed. Installation was without incident, and it went pretty fast.
Before completing the finishing touches, i.e. the molding, or whatever it's called, along the edges of the room, Mr. Moquin suggested that we use plain white to go along with the baseboard. It dressed up the room better than the molding that we originally picked out; so, we went along with his suggestion. It looks great! The only downside is that we have to return the package of molding to the store, and we'll probably take a hit on a restocking fee.
They cleaned up as they went along, and the whole project took a little over two hours from start to finish.
We asked Mr. Moquin about how to maintain the floor, and he strongly recommended just using a dry dust mop, and absolutely no water.
He and his assistant helped us move some of the furniture back into the family room. AND they hauled away the carpet and padding.
The price for his services was included in the purchase price of the flooring, and the price of the flooring was $1,410.82.

My comment regarding the hit on the restocking fee was not intended as a criticism. As Mr. Moquin pointed out to us, even with the restocking fee we're ahead of the game. We didn't mean it to be a negative -- it was just a statement of what took place, nothing more.
We're planning another project, and we intend to ask that Mr. Moquin be the one who does the installation.
"The only thing that I could add to this is that what's being mentioned as a downside, some may consider an upside. Due to his white baseboards I offered that white quarter round would look better than the colored quarter round that he had already bought from a third party. Since I had enough white quarter round in my truck, I offered it to him for free leaving him the opportunity to return his to the third party for refund. If the third party deducts a 15% restocking charge and I gave him the moldings that he used for free, then some may say that he got all his moldings at 85% off. My service was a labor only service."
