After inspecting the problem the plumber informed me that the cost to fix would be $175. I agreed and he subsequently attempted to replace the wax ring. He realized at this point that the flange was broken and the subfloor was wet/damaged and couldn't hold a bolt. He needed to bring in a carpenter to replace a section of the subfloor surrounding the toilet and also to replace the ceiling sheetrock below. He then recommended someone who he does a lot of business with. We had to reschedule two weeks out to find a day in which they were both available.
The new price from the plumber was $675 (including the previous visit) to pull the toilet, replace the flange after the new subfloor was installed, and then re-install the toilet, plus fix a leak at a shut-off valve and disconnect the refrigerator ice maker connection. I thought this was a little steep but as I'm managing all of this remotely I had to rely on him being honest.
I received a call from my tenant after the plumber left indicating that the toilet was not working. The plumber then went back the next day and determined that something happened with the fill valve while he was performing his work. He replaced the fill valve at no additional cost and now the toilet works. I would've expected him to ensure the toilet was functioning properly before leaving the job, but nevertheless he did make it right in the end.
The plumber was very responsive through phone calls and texts. He sent pictures of progress and the work seems to be quality. The only thing I found strange was that I was asked to send payments to the plumber at his home address rather than to the Quinn Plumbing business address. Perhaps this was handled as a side job. I don't really care as long as the job got done professionally and the problems were fixed.
I would call them again with any issues in the future.
