New GAF roof shingles installed by Lifetime Construction failed two city inspections before shingle manufacture company rep ok'd removal and replacement of roof installed by Lifetime Construction. That replacement passed inspection. Installation crews seemed to be day workers, only one person on crew spoke English. Cause of first application of new roof was thought to be either defective shingles, improper installation or "weather changes" during installation. No specific cause of failure was provided.
